Duplicate Permanent Registration
1. The application will be made online through MP Online portal or through Public Service Centre / Kiosk.
2. Candidate has to create their user profile/account on Madhya Pradesh Medical Council (mponline.gov.in) for the registration process and the candidate should submit the application through his user profile only.
3. The Permanent registration of the applicant should be alive in Madhya Pradesh Medical Council.
4. Required Documents:
o SSC / 10th Marksheet (Date of Birth proof)
o HSC/ 12th Marksheet
o All Marksheet of MBBS final part – 1
o All Marksheet of MBBS final part – 2
o Internship completion Certificate
o MBBS Degree
o Certificate of Permanent Registration of Madhya Pradesh Medical Council (if its copy is available)
o Copy of FIR report to the concerned police station in case of loss/theft/destruction of registration certificate..............Read more[....]
